Job summary

Qualifications

• High School diploma or equivalent.
• Current, valid Wisconsin (LPN) nursing license.
• At least one year experience as an LPN, preferably in a skilled nursing environment.
• BLS/CPR certification.
• Read, write and comprehend the English language.

Responsibilities

• Receives verbal report and communicates all pertinent resident care information to the CNAs on assigned unit or area.
• Visits each resident to ensure the appropriate cares are being provided, reports any problems to the manager/supervisor.
• Administers and documents medication and treatments, etc. within the parameters of the LPN licensure and according to the care plan. Reports any changes to the manager/supervisor.
• Reports significant changes in residents’ conditions immediately to the manager/supervisor and notifies appropriate people as directed.
• Maintains timely, accurate written charts, documents, records as required by law and/or regulation policies and procedures.
• Review and revise care plans as changes occur to reflect the exact condition of each resident.

Ovation Communities is seeking a Licensed Practical Nurse (LPN) to providegeneral nursing care, including treatments, administering of medication to residents, and preparing and assessing examinations. The LPN provides personal care services to nursing home residents to meet individuals spiritual, physical, emotional and social needs consistent with the plan of care, Standard Nursing practice, scope of gerontological nursing procedures and all applicable regulations, codes, policies and procedures
