Ovation Communities in Milwaukee, WI seeks a Licensed Practical Nurse (LPN) to provide general nursing care to residents, including medication administration and care planning under the established care plan. The LPN supports residents’ spiritual, physical, emotional and social needs in line with regulations and standards.

Shift: AM. Requires a Wisconsin LPN license, BLS/CPR, and at least 1 year of LPN experience in a skilled nursing setting.

Formación

• Current Wisconsin LPN license
• At least 1 year experience as LPN (preferably in skilled nursing)
• BLS/CPR certification
• Read, write and comprehend English

Responsabilidades

• Receives verbal report and communicates resident care information to CNAs
• Visits residents to ensure appropriate cares are provided and reports problems
• Administers and documents medication and treatments within licensure and care plan
• Reports significant changes in residents' conditions to manager/supervisor
• Maintains timely written charts, documents, records as required by law
• Review and revise care plans as changes occur

Location: Ovation Communities - Chai Point

Ovation Communities is seeking a Licensed Practical Nurse (LPN) to provide general nursing care, including treatments, administering of medication to residents, and preparing and assessing examinations. The LPN provides personal care services to nursing home residents to meet individuals spiritual, physical, emotional and social needs consistent with the plan of care, Standard Nursing practice, scope of gerontological nursing procedures and all applicable regulations, codes, policies and procedures

Key Responsibilities

• Receives verbal report and communicates all pertinent resident care information to the CNAs on assigned unit or area.
• Visits each resident to ensure the appropriate cares are being provided, reports any problems to the manager/supervisor.
• Administers and documents medication and treatments, etc. within the parameters of the LPN licensure and according to the care plan. Reports any changes to the manager/supervisor.
• Reports significant changes in residents' conditions immediately to the manager/supervisor and notifies appropriate people as directed.
• Maintains timely, accurate written charts, documents, records as required by law and/or regulation policies and procedures.
• Review and revise care plans as changes occur to reflect the exact condition of each resident.

Qualifications

• High School diploma or equivalent
• Current, valid Wisconsin (LPN) nursing license
• At least one (1) year experience working as an LPN, preferably in a skilled nursing environment
• BLS/CPR certification
• Read, write and comprehend the English language
