Job summary

Alvita Care in Eastern Connecticut seeks experienced LPNs for 6-, 8-, or 12-hour shifts, including day and night options. Roles are available per diem, part-time, or around existing schedules with 1:1 home care.

Shifts require driving to patient homes, delivering compassionate care, and documenting care plans in our system. CT LPN licensure, BLS/ACLS/CPR, and at least 1 year of LPN experience are expected.

Qualifications

• CT LPN licensure.
• BLS, ACLS, or CPR certification.
• Minimum of One (1) year demonstrated experience as an LPN.
• Ability to put patients at ease and explain physician's instructions.
• Ability to multi-task with excellent organization skills.
• Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
• Knowledge of medical terminology.
• Team player and ability to work independently.
• Valid driver's license or NYS Non-Driver photo ID card, required.
• Drivers preferred!

Responsibilities

• Consult with Management on complex/unique cases.
• Monitor/Assess patients/clients' condition, effects of the care plan.
• Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
• Manages, plans, coordinates and executes the patient's safe discharge from service.
• Ensures physician's orders, discharge summary and communications meet requirements.
• Communicates with patient's MD as needed and coordinates caseload.
• Assists patients with activities of daily living.
• Documents all services in internal system.
• Transport/ utilize medical equipment; travel to homes/facilities.
• Perform other duties and participate in special projects.

• Perform other related duties and participates in special project, as assigned.
