Job summary

Alvita Care is seeking experienced, CT-licensed Registered Nurses (RNs) to provide private duty nursing services in Western Connecticut (Fairfield, New Haven, Litchfield County). Ideal candidates are reliable, compassionate, and able to deliver consistent, one-on-one care in the home setting.

Shifts vary from 6, 8, to 12 hours with day and night options, and opportunities to perform skilled nursing visits. Part-time, per diem, or scheduling around existing commitments are available to fit your

Qualifications

• CT RN licensure required and valid in CT.
• 6 months to 1 year RN experience in a clinical setting.
• Ability to communicate clearly and explain physician's instructions.
• Valid driver''s license or Non-Driver photo ID; transportation preferred.

Responsibilities

• Assess needs of clients to develop a plan of care.
• Consult with management on complex/unique cases.
• Provide a plan of care with support of the multidisciplinary team to ensure services follow the plan of care.
• Monitor/assess patient/clients' condition and response to care.
• Oversee/supervise care provided by LPNs and paraprofessionals.
• Manage, plan, and coordinate patient discharge from service.
• Ensure physician orders and discharge communications meet regulatory requirements.
• Communicate with patient''s MD and family as needed; coordinate caseload aspects.
• Assist patients with activities of daily living.
• Document services in internal system accurately.
• Transport and travel to patient homes and facilities.

QUALIFICATIONS:

• CT RN licensure.
• BLS, ACLS, or CPR certification.
• Minimum of 6months to (1) year demonstrated experience as an RN
• Ability to put patients at ease and explain physician's instructions.
• Ability to multi-task with excellent organization skills.
• Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
• Knowledge of medical terminology.
• Team player and ability to work independently
• Valid driver's license or Non-Driver photo ID card
• Drivers preferred!

ESSENTIAL DUTIES & RESPONSIBILITIES:

• Assessments needs of the clients to develop a plan of care
• Consult with Management on complex/unique cases
• Provide a plan of care with support of the Clinical/Medical/appropriate multidisciplinary members to ensure the services follow the plan of care by assessment, planning, coordination, counseling, teaching and supervision to meet needs of the clients/patients/.
• Monitor/Assess patients/clients' condition, effects of the care plan,
• Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
• Manages, plans, coordinates and executes the patient's safe discharge from service.
• Ensures that physician's orders, discharge summary and communication to team members, patient and family members adhere to company and regulatory requirements.
• Communicates and consults with patient's MD with all manners when needed, manage and coordinate all aspects of assigned caseload.
• Assists patients with activities of daily living
• Documents content and process of all services provided in internal system properly.
• Transport/utilize medical equipment to and from clients/patients' home/facilities. Travel to patient/clients' homes and/or other facilities.
• Perform other related duties and participates in special project, as assigned.
