Solaris HealthCare Charlotte Harbor is a 180-bed Skilled Nursing and Rehab Facility in Port Charlotte, Florida. We are looking for caring LPNs to provide outstanding care to our residents and support a dynamic team on weekend doubles.

We offer shift differentials, full/part-time opportunities, and a comprehensive benefits package including health, dental, vision, 401k, and tuition reimbursement. Florida AHCA background screening is required.

Qualifications

• LPN license required by the state of Florida.
• CPR certification required.
• Two years of nursing experience preferred.

Responsibilities

• Admissions, discharges and transfers of residents.
• Medication requisitions with pharmacy and physicians; medication administration.
• Adheres to infection control, emergency procedures and center policies.
• Collaborates with the interdisciplinary team to achieve the best outcomes for residents.
• Identifies and reports abnormal changes in residents' condition to the appropriate personnel.
