Job summary

Blue Ridge Care is seeking a compassionate Licensed Practical Nurse (LPN) for hospice care. The role is available full-time, part-time, or PRN, providing direct patient care under RN supervision and contributing to individualized care plans that address comfort and dignity.

You will work with an interdisciplinary team to meet physical, emotional, and spiritual needs, documenting care in the EMR and educating patients, families, and caregivers to promote understanding and comfort.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Current LPN license in Virginia with CPR certification.
• At least two years of nursing experience in hospice, community health, or medical-surgical settings.

Responsibilities

• Provide direct patient care under the supervision of a Registered Nurse according to the plan of care.
• Document care accurately in the EMR at the bedside.
• Educate and support patients, families and caregivers to promote comfort and understanding.
• Collaborate with interdisciplinary team to address physical, emotional and spiritual needs.
• Manage equipment and supplies; communicate changes to RN or provider.
• Participate in team meetings and ongoing professional development.

About the Role

We’re seeking a compassionate Licensed Practical Nurse (LPN) to join our hospice team—available full-time, part-time, or PRN. As an LPN, you’ll provide direct patient care under the supervision of a Registered Nurse, following individualized care plans and ensuring every patient receives the highest standard of comfort and support. You’ll collaborate closely with an interdisciplinary team to deliver care that meets physical, emotional, and spiritual needs.

• Provide direct patient care as outlined in the plan of care established by the Registered Nurse.

• Implement nursing interventions based on assessments and care plans.

• Document care accurately and timely in the EMR—right at the bedside.

• Educate and support patients, families, and caregivers to promote comfort and understanding.

• Provide appropriate pain and symptom management, and communicate changes to the RN or provider.

• Participate in team meetings, interdisciplinary discussions, and ongoing professional development.

What You’ll Bring
