Job summary

Blue Ridge Care is seeking a compassionate LPN or RN for Extended Hours Hospice Care on a PRN basis. You will respond to urgent needs after-hours, perform nursing assessments, deliver direct hospice care, and coordinate with the interdisciplinary team to support comfort and dignity.

This role requires travel throughout the Blue Ridge Care territory and may include evenings, overnights, weekends, and holidays, with flexible scheduling and applicable shift differentials to support patient-focused

Qualifications

• Current Virginia RN or LPN license.
• Graduate of an accredited nursing program.
• Hospice, home health, or palliative care experience preferred.
• CPR certification required.
• Strong assessment and communication skills.

Responsibilities

• Respond to emergent calls during extended coverage hours.
• Provide urgent visits for symptom changes and end-of-life concerns.
• Deliver skilled nursing care per patient goals and orders.
• Assess patient conditions and communicate with the care team.
• Administer medications and treatments as prescribed.
• Support symptom management and patient comfort.
• Educate patients and families on disease progression and care techniques.
• Provide emotional support in challenging situations.
• Coordinate equipment, supplies, and services for home care.

About the Role

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) – Extended Hours Hospice Care to join our hospice team on a PRN (as-needed) basis.

This role provides coverage during extended hours, helping ensure patients and families receive timely clinical support when urgent needs arise outside of regular business hours. Shifts may vary based on organizational needs and may include evenings, overnight hours, weekends, holidays, or other periods requiring additional clinical coverage.

In this role, you will respond to patient and family needs, complete nursing assessments, provide direct hospice care, and collaborate with the interdisciplinary team to promote comfort, dignity, and quality of life.

This position travels throughout the entire Blue Ridge Care service area.

Schedule

How You'll Make a Difference

• Respond to emergent calls from patients and families during extended coverage hours.

• Provide urgent visits to address symptom changes, patient needs, or end-of-life concerns.

• Deliver skilled nursing care in accordance with patient goals and physician orders.

• Assess and evaluate patient conditions and communicate recommendations to the care team.

• Administer medications and treatments as prescribed.

• Support symptom management and patient comfort.

• Educate patients and families on disease progression, symptom management, and care techniques.

• Provide emotional support and reassurance during difficult and often unexpected situations.

• Communicate significant changes in condition to physicians and obtain necessary orders.

• Complete timely and accurate clinical documentation.

• Coordinate equipment, supplies, and services needed to support quality care in the home.

• Collaborate with the interdisciplinary team to ensure continuity of care.

• Assist with crisis visits, death visits, admissions, and other urgent hospice needs as appropriate.

• Promote a safe and supportive environment for patients and families.

• Participate in ongoing education and quality improvement activities as applicable.

What You'll Bring

For RN Candidates

• Current Virginia Registered Nurse (RN) license.

• Graduate of an accredited nursing program.

• Hospice, home health, ICU, critical care, or related nursing experience preferred.

For LPN Candidates

• Current Virginia Licensed Practical Nurse (LPN) license.

• Graduate of an accredited practical nursing program.

• Hospice, home health, long-term care, or related clinical experience preferred.

For All Candidates

• Current CPR certification.

• Strong assessment, communication, and clinical decision‑making skills.

• Knowledge of hospice philosophy and end‑of‑life care principles.

• Knowledge of Medicare and Medicaid hospice regulations preferred.

• Proficiency with electronic medical records.

• Ability to travel throughout the Blue Ridge Care territory.

• Ability to work independently and respond effectively in urgent situations.
