Job summary

Blue Ridge Care is seeking a compassionate Licensed Practical Nurse (LPN) to join our hospice team. You will provide direct patient care under RN supervision, following individualized plans of care to ensure comfort and dignity for patients in their most vulnerable moments.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Current LPN license in Virginia.
• CPR certification and at least two years of nursing experience (community health, hospice, or medical-surgical preferred).
• Excellent communication, organization, and critical thinking skills.

Responsibilities

• Provide direct patient care under RN supervision.
• Implement nursing interventions based on assessments and care plans.
• Document care accurately and timely in the EMR at the bedside.
• Educate and support patients, families, and caregivers to promote comfort.
• Manage equipment and medical supplies efficiently.
• Provide pain and symptom management and communicate changes to the RN or provider.
• Participate in team meetings and ongoing professional development.

About The Role

We're seeking a compassionate Licensed Practical Nurse (LPN) to join our hospice team. As an LPN, you'll provide direct patient care under the supervision of a Registered Nurse, following individualized care plans and ensuring every patient receives the highest standard of comfort and support. You'll collaborate closely with an interdisciplinary team to deliver care that meets physical, emotional, and spiritual needs.
