Blue Ridge Care is seeking a compassionate Licensed Practical Nurse (LPN) to join our hospice team. The role offers full-time, part-time, or PRN schedules, with direct patient care under RN supervision and collaboration with an interdisciplinary team to address physical, emotional, and spiritual needs.

Qualified candidates hold an accredited practical nursing program diploma, Virginia LPN licensure, CPR certification, and at least two years of nursing experience.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Current LPN license in Virginia.
• CPR certification and at least two years of nursing experience (community health, hospice, or medical-surgical preferred).

Responsibilities

• Provide direct patient care as outlined in the plan of care established by the Registered Nurse.
• Implement nursing interventions based on assessments and care plans.
• Document care accurately and timely in the EMR—right at the bedside.
• Educate and support patients, families, and caregivers to promote comfort and understanding.
• Manage equipment and medical supplies efficiently.
• Provide appropriate pain and symptom management, and communicate changes to the RN or provider.
• Participate in team meetings, interdisciplinary discussions, and ongoing professional development.
• Support the mission, vision, and values of Blue Ridge Care every day.

About the Role

We’re seeking a compassionate Licensed Practical Nurse (LPN) to join our hospice team—available full-time, part-time, or PRN. As an LPN, you’ll provide direct patient care under the supervision of a Registered Nurse, following individualized care plans and ensuring every patient receives the highest standard of comfort and support.
