Lifepoint Health’s Ascension Saint Thomas Rehabilitation Hospital in Nashville, TN is hiring a Licensed Practical Nurse (LPN) to join our care team. This full-time night-shift role offers a generous $8,000 sign-on bonus and comprehensive benefits.

You will administer medications, support care planning, and collaborate with an interdisciplinary team to advance patient recovery. Requirements include graduation from an accredited LPN program, active TN LPN license, CPR/BLS, and 6 months Med/Surg

Qualifications

• Graduate of an accredited Licensed Practical/Vocational Nurse program.
• Current LPN licensure in the state.
• CPR/BLS certification required; IV certification preferred or within 6 months.
• Six months Med/Surg experience in an acute care setting preferred.

Responsibilities

• Administer medications safely and accurately per patient needs.
• Assist RN with assessments and care planning.
• Identify rehabilitation needs and implement nursing interventions.
• Deliver care that supports patient safety and comfort.
• Educate patients and families with individualized teaching plans.
• Collaborate with interdisciplinary team to update care plans.
• Perform timely QI assessments under RN supervision.
• Perform other duties as assigned.

What We're Looking For

The Licensed Practical Nurse (LPN) provides hands-on nursing care under the supervision of a Registered Nurse (RN). This includes delivering treatments, monitoring patient progress, supporting care planning, and ensuring safety and comfort in alignment with individualized patient needs.

Essential Functions

• Administer medications safely, accurately, and based on patient age and clinical condition.
• Minimize potential for medication errors by following proper medication administration procedures.
• Assist the RN with initial assessments and patient reassessments per policy.
• Identify and implement appropriate rehabilitation nursing interventions.
• Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
• Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
• Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
• Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
• Develop and implement individualized teaching plans based on patient and family learning needs.
• Perform other duties as assigned.

Applicants should have a current state LPN license from an accredited nursing school. Additional requirements include:

• Education: Graduate of an accredited Licensed Practical/Vocational Nurse program.
• Experience: Minimum of six months of Med/Surg experience in an acute care setting preferred.
• License: Current state licensure as a Licensed Practical/Vocational Nurse.
• Certifications: CPR/BLS certification required; IV Certification highly preferred or obtained within 6 months.
• Strong clinical skills and attention to detail.
• Compassionate approach and commitment to patient-centered care.
• Ability to work effectively within an interdisciplinary team.
• Dedication to continuous learning and professional development.
