Job summary

HSHS Rehabilitation Hospital, a Lifepoint Health facility, seeks an LPN for PRN shifts to deliver direct and indirect patient care in an inpatient rehab setting in Wisconsin. You will use the nursing process to assess, plan, intervene and evaluate patient needs.

You will collaborate with patients, families, and a wide interdisciplinary team to develop and adjust personalized care plans, monitor changes, and communicate with physicians and supervisors. Discharge planning is a key responsibility.

Qualifications

• Licensure as LPN is required.
• Graduation from an accredited nursing program (BSN/ADN/diploma).
• Strong communication and interpersonal skills.

Responsibilities

• Deliver direct and indirect patient care using the nursing process (Assessment, Planning, Intervention, Evaluation).
• Collaborate with patients, families, and interdisciplinary team to create personalized care plans.
• Monitor and communicate changes in patient conditions to physicians and staff.

How You'll Contribute

Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plan in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient's clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate. Participates in discharges planning process.

What We're Looking For

Applicants should possess the following qualifications and skills:

• Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse.
• CPR/BCLS certification. ACLS preferred.
• Minimum six months' Medical/Surgical experience in an acute care setting preferred.
• Excellent oral and written communication and interpersonal skills.
