Lifepoint Health's Mercy Rehabilitation Hospital South is seeking a Licensed Practical Nurse for a part-time day schedule in St. Louis, MO. You will provide direct and indirect patient care, work with an interdisciplinary team, and help with discharge planning to support safe transitions.

This role emphasizes compassionate care, strong communication, and collaboration to improve patient outcomes in an acute rehab setting.

Qualifications

• Graduation from an accredited nursing program (BSN/ADN or Diploma).
• Current state licensure as Licensed Practical Nurse.

Responsibilities

• Deliver direct and indirect patient care using the nursing process: assessment, planning, intervention and evaluation.
• Collaborate with patients, families and interdisciplinary care team to create care plans.
• Monitor patient conditions and communicate changes to physicians and supervisors.
• Play a key role in discharge planning to support safe transitions.

How You'll Make An Impact

As a Licensed Practical Nurse, you'll be a trusted partner in our patients' recovery journeys. You will:

• Deliver direct and indirect patient care using the nursing process-Assessment, Planning, Intervention, and Evaluation
• Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans
• Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues

Qualifications And Requirements

• Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse.

Additional Requirements Include

• CPR/BCLS certification. ACLS preferred.
• Minimum six months' Medical/Surgical experience in an acute care setting preferred.
• Excellent oral and written communication and interpersonal skills.
