Mercy Rehabilitation Hospital - Springfield, part of Lifepoint Health, is seeking a Full-time Licensed Practical Nurse (LPN) to join our compassionate care team. You will provide direct and indirect patient care using the nursing process and collaborate with an interdisciplinary team to develop personalized care plans.

This role supports discharge planning and ongoing patient education in a setting that emphasizes work-life balance, professional growth, and a patient-centered culture.

Qualifications

• Graduation from an accredited BSN/ASN or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse (LPN).
• CPR/BCLS certification; ACLS preferred.
• Minimum six months' Medical/Surgical experience in acute care preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Deliver direct and indirect patient care using the nursing process (Assessment, Planning, Intervention, Evaluation).
• Collaborate with patients, families and interdisciplinary care team to create personalized care plans.
• Monitor and communicate changes in patient conditions to physicians, supervisors and colleagues.
• Play a key role in discharge planning for safe, successful transitions.
