Job summary

HSHS Rehabilitation Hospital, part of Lifepoint Health, Inc., seeks a Licensed Practical Nurse for rehab services in Wisconsin. This full-time day position follows a 3x12-hour shift pattern, with a sign-on bonus up to $7,500. Requires Wisconsin LPN license, BLS within 30 days, and an Associate's degree.

The role includes direct and indirect patient care, care planning with families, and active participation in discharge planning and interdisciplinary collaboration to ensure safe transitions.

Qualifications

• AHA Basic Life Support (BLS) within 30 days of hire.
• Critical thinking, decisive judgement, and ability to work with minimal supervision.
• One year of prior nursing experience preferred.
• Active Wisconsin or compact state LPN license required; PSOR updates if relocating.
• Associate's Degree in nursing or related field required.

Responsibilities

• Provides planning and delivery of direct and indirect patient care through the nursing process: Assessment, Planning, Intervention, and Evaluation.
• Develops nursing care plans in coordination with patient, family and interdisciplinary staff.
• Communicates changes in patient condition with Physicians and Nursing leadership.

How you'll Contribute

Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plan in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient's clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate. Participates in discharges planning process.

• Deliver direct and indirect patient care using the nursing process-Assessment, Planning, Intervention, and Evaluation


• Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans


• Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues


• Play a key role in the discharge planning process to support safe, successful transitions

What we are looking for:

• Certifications: AHA Basic Life Support (BLS) within 30 days of hire


• Required Skills: Requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.


• Experience: One year previous experience preferred


• Licenses: Active Wisconsinor compact state LPN license required. If relocating, licensure must be updated to new Primary State of Residence (PSOR) before employment begins.


• Education: Associate's Degree required.
