Grant Healthcare and Rehabilitation is seeking a full-time evening shift LPN/RN to join our team. The 3p-11p schedule offers an active role in patient care within a supportive facility.

Responsibilities include bedside care, vitals, medication administration, wound care, and accurate charting. We provide quarterly raises, comprehensive benefits, tuition reimbursement, a 401K plan, shift differentials, daily pay, and uniforms.

Qualifications

• Active LPN or RN license in Kentucky.
• CPR certification and first aid.
• Willingness to work evenings and weekends.
• Excellent staff training and customer service skills.

Responsibilities

• Provide bedside care to patients, including personal hygiene maintenance.
• Record the patients’ vital signs (BP, pulse, respirations).
• Collect patients’ health information and maintain accurate records.
• Relay information between patients, RNs, and physicians.
• Administer oral medications and injections as prescribed.
• Test and monitor blood sugar levels.
• Assist with wound care and dressing changes.
• Monitor for changes in patient condition and report promptly.
• Maintain confidentiality of patient records and proper documentation.
• Rotate holidays as required.

LPN/RN responsibilities:

• Provide bedside care to patients, including personal hygiene maintenance
• Record the patients’ vital signs (including blood pressure, pulse, and respiratory rate)
• Collect the patient’s health information
• Relay information between patients, registered nurses, and physicians
• Give oral medications and injections to patients
• Test a patient’s blood sugar levels
• Urinary catheter insertion and care
• Wound care and dressing change
• Assist patients with personal hygiene
• Monitor for changes in patients
• Proper charting, documentation, and maintenance of patients’ medical records
• Asks questions when in doubt for clarification by the Registered Nurse.
• Acts as a role model for ancillary personnel.
• Maintains the confidentiality of the patient’s clinical records.
• Rotating holidays required.

LPN/RN qualifications:

• Up-to-date first aid and CPR certification
• Current LPN or RN license in the state
• Willingness to work evenings and weekends
• Exceptional staff training and customer service skills
• Compassionate and friendly demeanor
