Job summary

Fair Oaks Health and Rehabilitation is seeking compassionate LPNs/RNs to join our night shift team. You will deliver high-quality, resident-centered care and support daily clinical needs in a collaborative, team-oriented setting.

Qualifications

• Current LPN or RN license.
• At least one year of LPN or RN experience in a long-term care setting preferred.

Responsibilities

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans.
• Prepares and administers medications and records prescribed treatments; reports adverse reactions as required.
• Records patients' medical information and vital signs.
• Monitors equipment and interprets electronic displays to ensure patient safety.
• Initiates corrective action when a patient shows adverse symptoms.
• Provides bedside care for a wide variety of medical patients.
• Changes dressings, inserts catheters, and starts IVs as needed.
• Supports discharge planning and collaborates with the interdisciplinary team.

Education

Current LPN or RN license

1 year LPN/RN LTC experience preferred

LPN/RN Core Responsibilities:

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans
• Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications. Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse.
• Records patients' medical information and vital signs.
• Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays.
• Initiates corrective action whenever the patient displays adverse symptomatology
• Provides bedside care for a wide variety of medical patients.
• Changes dressings, inserts catheters, and starts IVs
• Prepares equipment and aids physician during the examination and treatment of patient
• Responds to life-saving situations based upon nursing standards and protocol
• Participates in discharge planning.
• Utilizes time management skills.
• Records all care information concisely, accurately, and completely, in a timely manner, in the appropriate format, and on the appropriate forms.
• Performs other position-related duties as assigned, depending on the assignment setting

LPN/RN Requirements:

• Current LPN or RN license
• At least one year of LPN or RN experience in a long-term care setting is preferred
