Job summary

Fair Oaks Health and Rehabilitation in Kentucky is seeking dedicated LPNs and RNs for our full-time night shift to provide compassionate, resident-centered care and support daily clinical needs.

You will work with an interdisciplinary team to ensure comfort, safety, and well-being, manage medications, monitor vital signs, and participate in discharge planning while upholding quality care standards.

Qualifications

• Current LPN or RN license.
• At least one year of LPN or RN experience in a long-term care setting is preferred.

Responsibilities

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans.
• Prepares and administers medications and records prescribed treatments.
• Records patients' medical information and vital signs.
• Monitors equipment used on patients and interprets electronic displays.
• Initiates corrective action when adverse symptoms occur.
• Provides bedside care for a wide variety of medical patients.
• Changes dressings, inserts catheters, and starts IVs.
• Assists during examinations and treatments.
• Responds to life-saving situations according to nursing standards and protocol.
• Participates in discharge planning.
• Records care information concisely and accurately.

LPN/RN Core Responsibilities:

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans

• Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications. Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse.

• Records patients' medical information and vital signs.

• Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays.

• Initiates corrective action whenever the patient displays adverse symptomatology

• Provides bedside care for a wide variety of medical patients.

• Changes dressings, inserts catheters, and starts IVs

• Prepares equipment and aids physician during the examination and treatment of patient

• Responds to life-saving situations based upon nursing standards and protocol

• Participates in discharge planning.

• Utilizes time management skills.

• Records all care information concisely, accurately, and completely, in a timely manner, in the appropriate format, and on the appropriate forms.

• Performs other position-related duties as assigned, depending on the assignment setting

LPN/RN Requirements:

• Current LPN or RN license

• At least one year of LPN or RN experience in a long‑term care setting is preferred
