Responsibilities and Duties:

• Provide nursing care in accordance with physician orders, agency policies, and Oregon regulations
• Conduct routine nursing visits and monitor client health status
• Perform medication management and medication set-up as permitted by agency policy and state regulations
• Provide wound care and other nursing services within LPN scope of practice
• Observe, document, and report changes in client condition to the supervising RN
• Educate clients and families on health maintenance and safety measures
• Complete accurate and timely documentation

Requirements:

• Current TB Screen and Criminal Record Check
• Must be able to legally work in Oregon, USA to apply for this position
• Home health, long-term care, assisted living, or acute care experience preferred
• Strong assessment, communication, and documentation skills
• Current CPR certification
• Valid driver’s license, proof of insurance, and reliable transportation
• Ability to work independently and manage a flexible schedule
