Sabel Company seeks a Licensed Practical Nurse (LPN) to join our care team. You will manage admissions, discharges and transfers, coordinate medications with pharmacies and physicians, and ensure safety per center policies.

Qualifications

• LPN licensed by the state where practice occurs (appropriate license verification).
• CPR certification required or current at time of hire.

Licensed Practical Nurse (LPN) Responsibilities:

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergency, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy

Licensed Practical Nurse (LPN) Skills and Qualifications:

• LPN (Licensed Practical Nurse) licensed by the State
• CPR Certification required
• Two Years prior nursing experience preferred
• Valid NYS LPN license
• Previous experience in long-term care or a skilled nursing facility is preferred
• Strong communication and organizational skills
• Compassionate and dependable with a team-first attitude
