Job summary

UCP Long Island is seeking supportive, dependable LPNs for a residential children's program in Central Islip, NY. Under RN supervision, you will assist in the overall health and safety of the individuals served.

Shifts include 8:00am-4:00pm, 4:00pm-12:00am, and 12:00am-8:00am, with responsibilities such as medication administration, daily health monitoring, and coordination of medical appointments. UCP Long Island is a not-for-profit celebrating 75+ years.

Qualifications

• Associate's degree or equivalent and LPN/Licensed Practical Nurse certification.
• Experience with medication administration and health monitoring preferred.
• Ability to document accurately and communicate with RN and care staff.

Responsibilities

General Purpose of the Job

Under the supervision of a Registered Nurse, assist in the overall health and safety of the individuals served.

Essential Duties and Responsibilities

include the following. Other duties may be assigned.

• Knowledge of all medications for all individuals.


• Administer medications and treatments as prescribed by physician.


• Generate incident, seizure, and illness reports.


• Monitor and report health care on daily flow report records.


• Instruct mid-level supervisors and direct care staff in above, i.e., hygiene, diet, weights, etc.


• Submit written monthly, quarterly, annual nursing/medical progress notes.


• Coordinate laboratory care and medical appointments (routine, fees, special and emergency).


• Maintain functioning interpersonal relationships with other disciplines.


• Maintain ongoing communication with other programs, i.e., client health care.


• Monitor program participants during mealtimes.


• Order, record, and maintain sufficient/adequate supply of medication.


• Maintain sufficient/adequate amount of medical supplies/materials.


• Maintain accurate medical records including medication administration sheets, Kardex, Formulary,\"Pick-up\" and follow through on all physician orders.


• Provide emergency first aid. Coordinate emergency services.


• Supervise direct care staff in absence of mid-level supervisor.

Education and/or Experience

Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience.
