Job summary

SanStone Health & Rehabilitation is seeking a night-shift Licensed Practical Nurse (LPN) to join our dedicated care team in North Carolina. This full-time role operates 12-hour shifts from 7pm to 7am, and requires an unrestricted LPN license.

Qualifications

• LPN license required and in good standing.
• Experience in long-term care encouraged but not mandatory.

Responsibilities

• Supervise Certified Nursing Assistants and other staff.
• Coordinate nursing care for residents on your unit.
• Provide safe medication-related interventions and document orders.
• Transcribe and implement physician orders accurately.
• Perform nursing assessments and notify physicians of changes.
• Develop and adjust care plans based on assessments.
• Introduce appropriate nursing interventions to stabilize residents.

Under the direction and supervision of the Director of Nursing, the LPN will plan, coordinate, provide and manage nursing care, services, and health education to our residents. These positions require patience, compassion, and a desire to care for the residents in a gentle and empathetic manner.

Responsibilities may include:

• Supervise the care/services provided by Certified Nursing Assistants and other staff.


• Coordinate the nursing care of residents on the unit, assuring all assigned staff provide appropriate care to them.


• Provide safe and accurate medication related interventions to residents.


• Accurately transcribe and coordinate the implementation of physician orders.


• Perform nursing assessments regarding the health status of residents. Notify the physician of changes in status and promptly implement and document new orders.


• Develop a plan of care based on assessment, implementing nursing care.


• Select and institute appropriate nursing interventions that might be required to stabilize a resident's condition and /or prevent complications.


• Contribute to the resident's assessment and the development of a care plan.
