Job summary

ACG Hospice in Lexington, SC is seeking a dedicated Licensed Practical Nurse (LPN) to join our community hospice house on a full-time night shift (7pm–7am). You will work under the supervision of the registered nurse to visit patients and deliver routine care, while coordinating with the care team and families.

Qualifications

• Heart for serving patients and families in hospice care.
• Current state license as an LPN in good standing.
• 2+ years nursing experience; hospice experience preferred.

Responsibilities

• Conduct patient visits and provide routine care under RN supervision.
• Communicate with the care team, patients and their families to coordinate care.
• Support ongoing patient comfort and adherence to care plans.

Full-time night shift (7pm-7am).

We are looking for hospice licensed practical nurses (LPNs) who are committed to creating meaningful patient experiences. As a hospice licensed practical nurse (LPN), you’ll work under the direction and supervision of the registered nurse to conduct patient visits and provide routine care, while communicating with the care team, the patient and their family.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care


• Education: Graduate of an accredited school of nursing with a current state license as a licensed practical nurse


• Experience: 2+ years of nursing experience in an acute care setting, or equivalent experience (hospice experience preferred)


• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100lbs and bear the weight of an average adult effectively.
