As an LPN with Traditional Home Health and Hospice, you are able to give each individual patient your undivided attention as you care for them.
Job Title: Licensed Practical Nurse (LPN) - Hospice
Shift: Full-time, part-time, and per diem schedules available. Our office hours are Monday thru Friday 8:30am to 5:00pm.
Job Location: Clarks Summit, PA and surrounding areas.
Our nurses see patients in the most ideal setting - where the patient resides!
At Traditional, we do our best to keep each LPN's case load within a sensible geographic territory.

Job responsibilities include, but not limited to the following:

• Assesses patient status, administers patient care, personal care, and teaching of the patient in accordance with physician orders.
• Reports patient status changes, care, and progress to physician and nursing supervisor on a continuous basis.
• Timely and accurately prepares, maintains and submits meticulous documentation as required by Traditional Home Health Care including all interactions with patients, case conferences, medication changes, and various OASIS time points.
• Informs patients of other services that are available and makes referrals to other community resources as needed.

Requirements:

• Current license as a Licensed Practical Nurse (LPN) in Pennsylvania
• Must have current driver's license
• Ability to travel to case assignments
• Excellent assessment skills, documentation skills and ability to meet deadlines
• Ability to work independently with minimal supervision and manage time effectively
• Strong interpersonal skills.
• Demonstrates good verbal and written communication, and organizational skills
• Acute care and/or home health experience is preferred.
• Having experience with hospice patients or in the hospice environment is a plus.
• Understanding of Hospice philosophy and principles of death and dying.
