On-call Licensed Practical Nurse (LPN):

We are seeking an On-call Hospice Licensed Practical Nurse (LPN) to serve our patients. The position involves working 10 on-call days per month.

Position Summary:

The LPN provides skilled nursing care to hospice patients according to individualized care plans developed with the patient, family, physician, and hospice team. Care is provided where the patient resides, including home, facility, or hospital.

Qualifications:

• Current Pennsylvania LPN license
• Preferred minimum of two years of patient care experience
• Excellent clinical skills
• Strong organizational skills with attention to detail
• Outstanding written and verbal communication skills
• Reliable transportation with a valid Pennsylvania driver’s license and appropriate insurance
