Job summary

Senior Care Solutions is seeking PRN Licensed Practical Nurses (LPNs) to join our Medicare-certified home health team in Florida. LPNs provide skilled nursing visits in patients' homes under RN supervision, with a focus on compassionate, patient-centered care and flexible scheduling.

Ideal candidates hold a Florida LPN license, BLS/CPR, and at least 1 year of nursing experience; home health is preferred but not required. A valid driver’s license and reliable transportation are required.

Qualifications

• Current Florida Licensed Practical Nurse (LPN) license in good standing.
• 1+ year of nursing experience preferred.
• Home health experience is preferred but not required.
• Current BLS/CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong communication, organization, and time management skills.
• Ability to work independently while providing compassionate, patient-centered care.

Responsibilities

• Provide skilled nursing visits in patients' homes in accordance with the plan of care.
• Perform nursing assessments and report changes in patient condition to the supervising RN.
• Administer medications and treatments as ordered by the physician.
• Educate patients and caregivers on disease management, medications, and safety.
• Complete accurate and timely documentation in the electronic medical record.
• Collaborate with physicians, case managers, therapists, and other interdisciplinary team members.
• Ensure compliance with Medicare Conditions of Participation and agency policies.
