Job summary

Senior Care Solutions is seeking a compassionate PRN Licensed Practical Nurse (LPN) to join our Medicare-certified home health team in Florida. You will deliver skilled nursing visits in patients' homes under RN direction, participate in care planning, and document all care in the EMR.

This role offers flexible scheduling and the opportunity to impact patients and families directly. Requirements include an active Florida LPN license, BLS/CPR, and at least 1 year of nursing experience; home

• Current Florida LPN license in good standing.
• 1+ year of nursing experience preferred.
• Home health experience preferred but not required.
• Current BLS/CPR certification.

Responsibilities

• Provide skilled nursing visits in patients' homes in accordance with the plan of care.
• Perform nursing assessments and report changes in patient condition to the supervising RN.
• Administer medications and treatments as ordered by the physician.
• Educate patients and caregivers on disease management, medications, and safety.
• Complete accurate and timely documentation in the electronic medical record.
• Collaborate with physicians, case managers, therapists, and other interdisciplinary team members.
• Ensure compliance with Medicare Conditions of Participation and agency policies.

Position Summary

As a PRN Home Health LPN, you will provide skilled nursing care to patients in their homes under the direction of a Registered Nurse and in accordance with physician orders. This role offers a flexible schedule while allowing you to build meaningful relationships with patients and their families.
