Job summary

Senior Care Solutions is seeking a compassionate PRN Licensed Practical Nurse (LPN) to provide in-home nursing care for Medicare patients in Melbourne and surrounding areas. You will perform skilled nursing visits, administer medications, educate patients and families, and document care in the electronic health record under the supervision of an RN.

Qualifications

• Current Florida Licensed Practical Nurse (LPN) license in good standing.
• 1+ year of nursing experience preferred.
• Home health experience preferred but not required.
• Current BLS/CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing visits in patients' homes as per plan of care.
• Perform nursing assessments and report changes to supervising RN.
• Administer medications and treatments as ordered by physician.
• Educate patients and caregivers on disease management, medications, and safety.
• Complete accurate documentation in the electronic medical record.
• Collaborate with physicians, case managers, and other team members.
• Ensure compliance with Medicare conditions of participation and agency policies.

Position Summary

As a PRN Home Health LPN, you will provide skilled nursing care to patients in their homes under the direction of a Registered Nurse and in accordance with physician orders. This role offers a flexible schedule while allowing you to build meaningful relationships with patients and their families.

Responsibilities

• Provide skilled nursing visits in patients' homes in accordance with the plan of care.
• Perform nursing assessments and report changes in patient condition to the supervising RN.
• Administer medications and treatments as ordered by the physician.
• Educate patients and caregivers on disease management, medications, and safety.
• Complete accurate and timely documentation in the electronic medical record.
• Collaborate with physicians, case managers, therapists, and other interdisciplinary team members.
• Ensure compliance with Medicare Conditions of Participation and agency policies.

Qualifications

• Current Florida Licensed Practical Nurse (LPN) license in good standing.
• 1+ year of nursing experience preferred.
• Home health experience is preferred but not required.
• Current BLS/CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong communication, organization, and time management skills.
• Ability to work independently while providing compassionate, patient-centered care.
