Job summary

Bridgeway Home Health is seeking a Licensed Practical Nurse (LPN) to join our full-time home health field team in Georgia. You will provide skilled nursing care in patients' homes, administer treatments as prescribed, and educate families in appropriate care.

You will document care timely and collaborate with the interdisciplinary team to ensure high-quality, compassionate services for patients in the Covington, Mansfield, Oxford area and surrounding communities.

Qualifications

• Graduate of an accredited Licensed Practical Nurse educational program.
• Current Licensed Practical Nurse (LPN) license within the state of practice.
• Current CPR certification.
• Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills.
• Reliable transportation, valid driver's license and current automobile insurance.

Responsibilities

• Provide one-on-one patient care in patients' homes as prescribed in the plan of care.
• Educate patients and families on care procedures and health maintenance.
• Timely documentation and collaboration with the interdisciplinary team.

Overview

As a home health LPN, you will be responsible for conducting one-on-one patient care and family teaching in the home setting. The LPN administers skilled nursing care to patients in their place of residence as prescribed and in accordance with the established plan of care. The LPN is also responsible for timely documentation and collaboration with the interdisciplinary team.
