Job summary

Bridgeway Home Health is hiring a Licensed Practical Nurse (LPN) for full-time home health care in the Gwinnett County area. You will deliver one-on-one nursing care in patients’ homes, following the plan of care and collaborating with an interdisciplinary team.

The role requires an accredited LPN program graduate, an active LPN license in the state, CPR certification, and strong organizational and communication skills.

Qualifications

• Graduate of an accredited LPN educational program.
• Current Licensed Practical Nurse (LPN) license in the state of practice.
• Current CPR certification.
• Excellent verbal, written, organizational, collaboration, multitasking and time-management skills.
• Reliable transportation, valid driver’s license and current automobile insurance.

Responsibilities

• Provide one-on-one patient care and family teaching in the home setting.
• Administer skilled nursing care to patients per established plan of care.

Overview

As a home health LPN, you will be responsible for conducting one-on-one patient care and family teaching in the home setting. The LPN administers skilled nursing care to patients in their place of residence as prescribed and in accordance with the established plan of care. The LPN is also responsible for timely documentation and collaboration with the interdisciplinary team.

Requirements

• Graduate of an accredited Licensed Practical Nurse educational program
