Elara Caring is seeking a Registered Nurse Home Health to support Rockford, IL and surrounding areas. You will assess patients, perform initial and ongoing assessments, and administer care per physician orders while ensuring privacy and safety.

Responsibilities include monitoring conditions, educating families, and acting on changes promptly. A valid RN license and at least 1 year of clinical experience are required; travel to patient homes is expected.
