Schedule and Shift:

Full Time, Days 8:00am - 4:30pm

Location:

Home Health visits in Jackson or Clay County

The Role:

Administer nursing care to patients according to physician's orders, under the supervision of a Registered Nurse. Provide intermittent, individualized nursing care, educate and supervise patients and caregivers, and ensure continuity of care. Perform technical nursing skills proficiently, promote team collaboration, execute physician's orders, and document according to agency policies. Conduct home visits as scheduled.

• Provide efficient and organized direct patient care under RN supervision.
• Collect data relevant to the patient's age and contribute to the care plan.
• Report observations, implement physician-directed care plan changes, and communicate effectively with the team.
• Verify and implement physician orders (co-signed by RN).

Qualifications:

Required:

• Vocation training from an accredited LPN program
• Active American Heart Association BLS certification
• Valid driver’s license with current insurance
• State Licensure as an LPN in Kentucky or a Compact State

Preferred:

• Clinical experience in Medical/Surgical nursing
