Job summary

Elara Caring is seeking a Licensed Vocational / Practical Nurse to support our home health patients. You will follow individualized care plans, document medications, and work with the care team to ensure smooth, coordinated care.

The role emphasizes compassionate, preventive care to improve patient outcomes in a home setting. Required is an active LPN license, a nursing program graduate, and at least 1 year of clinical experience; hospice/home health experience is a plus.

• Ability to frequently lift, push, pull, and support up to 50 pounds
• Experience in hospice or home health is preferred

Responsibilities

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Experience in a hospice or home health environment is preferred
