Interim HealthCare of the Twin Cities is seeking a Home Care Licensed Practical Nurse (LPN) to visit patients in their homes, coordinating care, managing medications, wound care, catheter care, ostomy education, and infection prevention.

This full-time position requires MN LPN licensure, CPR, and the ability to travel within the southwest metro area. Competitive pay, mileage reimbursement, PTO, 401(k), and comprehensive medical benefits are offered.

Qualifications

• Current license as Licensed Practical Nurse (LPN) in MN with active standing.
• 2 years experience as an LPN preferred.
• Valid driver's license, reliable transportation, and proof of auto insurance.
• CPR certification required.

Responsabilités

• Participate in ongoing interdisciplinary assessment of the patient.
• Provide services ordered by the physician as in the plan of care.
• Recognize and report changes in patient condition to RN.
• Contribute to development and evaluation of the plan of care with patient and caregiver(s).
• Counsel and educate patient and family on health needs.
• Complete timely clinical notes including patient progress.

Home Care Licensed Practical Nurse (LPN):

Hours:

Full-time | Days, Monday - Friday | There will be a weekend and holiday rotation. Work approximately one weekend every 4-6 weeks and 2 holiday's per year. The weekend and holiday rotation is dependent upon staffing levels.

Territory:

This position will work in the field helping patients in their home setting. Our office is in Roseville, MN and the general territory for this position would be the southwest metro.

A few must-haves for this position:

• Current license as an Licensed Practical Nurse (LPN) in the State of MN. License is active and in good standing.
• 2 years experience as an LPN is preferred.
• Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance are required.
• CPR Certification

Some functions of what you would do as an LPN:

• Participatein ongoing interdisciplinary assessment of the patient.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Recognizes and reports changes in the patient’s condition to the RN.
• Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Counsels and educates the patient and family in meeting healthcare related needs.
• Completes timely and accurate clinical notes including addressing patient’s progress.
