Interim HealthCare of the Twin Cities is seeking a Home Care Licensed Practical Nurse (LPN) to provide in-home nursing care to patients in the southeast metro area. The role emphasizes care coordination, disease and medication management, wound care, and infection prevention in patients’ homes.
You will work full-time days, with a weekend and holiday rotation, and must have a MN LPN license, CPR certification, and the ability to travel within the service area.
Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The LPN will provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.
Hours: Full-time | Days, Monday - Friday | There will be a weekend and holiday rotation. Work approximately one weekend every 4-6 weeks and 2 holiday's per year. The weekend and holiday rotation is dependent upon staffing levels.
This position will work in the field helping patients in their home setting. Our office is inRoseville, MN and the general territory for this position would be the southeast metro.
