Job summary

Personal Touch Home Care is seeking a compassionate LPN for full-time home health across Chesapeake, Suffolk, and Isle of Wight. You will provide direct patient care under RN supervision, implement care plans, and educate patients and families in their own homes.

The role emphasizes patient-centered care, teamwork with clinicians, and ongoing professional growth within a supportive company culture. Candidates should be licensed LPN/LVN with valid drivers license and reliable vehicle.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Preferably at least one year of nursing experience.
• Currently licensed as an LPN or LVN in the State.
• Demonstrates excellent verbal and written communication and organizational skills.
• Must be a licensed driver with a reliable and insured automobile.

Responsibilities

• Providing direct patient care as outlined in the State Nurse Practice Act.
• Implementing care plans initiated by our clinicians.
• Documenting patient care accurately and promptly.
• Educating patients and their families/caregivers on diagnoses and care plans.
• Collaborating with the team to coordinate home health services.
• Using equipment and supplies effectively and efficiently.
