Home Health Licensed Practical Nurse (LPN) – Helena, MT or Surrounding Area

As a recipient of federal funds through the VA and/or Medicare/Medicaid, Interim HealthCare is required to comply with federal health care regulations. Because marijuana is legally available in Montana but remains federally illegal, a 10-panel pre‑employment drug screening is required. Responsibilities

• Provide home‑based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN.
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care.
• Take patient vitals, conduct assessments, document progress and report changes to the RN supervisor.
• Assist the RN in carrying out the patient’s plan of care and teaching patient self‑care techniques.
• Educate family on the patient’s plan of care, medication and home safety.
• Collaborate with the interdisciplinary team to coordinate patient care and plan for discharge.

Qualifications

• Graduate of an accredited practical nursing program and hold an active LPN license in Montana.
• One year of LPN experience, ideally in home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing, strong communication skills and compassion.
