Athens Health and Rehabilitation is seeking an LPN for day shift to deliver direct nursing care in a home-like setting in Athens, AL. You will work with RNs, physicians, and the interdisciplinary care team to support rehabilitation goals while ensuring comfort, safety and dignity for residents.

Responsibilities include medication administration, monitoring health status, collaboration with the rehab team, and maintaining thorough documentation.

Qualifications

• Current and valid LPN license.
• Strong clinical skills including medication administration and wound care.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
• Compassionate, patient-focused approach to care.
• BLS/CPR certification required.

• Monitor, document, and report changes in residents’ health status, including vital signs.
• Assist residents with activities of daily living (ADLs) as needed.

• Provide emotional support and education to residents and families.
• Ensure a clean, safe environment by following infection control protocols.
