The Sheridan at Oak Brook, part of Senior Lifestyle Corporation, is seeking a Licensed Practical Nurse (LPN) for assisted living in Oak Brook, IL. This full-time role requires an active LPN license in the state and 1+ year of senior-care experience with CPR certification.

You will administer medications, participate in admissions, and help develop care plans while coordinating with the health care team and physicians. English proficiency and teamwork are essential.

Qualifications

Responsibilities

• Administer medications according to Federal & State regulations and company guidelines.
• Assist with admissions, including assessments and documentation.
• Develop or contribute to acute and long-term care/service plans.
• Monitor clinical data (e.g., wounds, infections) under supervisor guidance.
• Provide health-related counseling and respond to resident health changes.
• Notify physicians and families of health changes and document appropriately.
• Respond promptly to emergencies and coordinate with staff.
• Educate residents and staff and maintain confidentiality.

Salary starting at $34 per hour

• Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
• May be responsible for the admissions of new residents to include all admission assessments and documentation.
• Responsible for completing acute and long-term care plans or service plans.
• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Maintain confidentiality of residents’ and prospective residents’ information.
• May be responsible for discharges, assessments, and clinical documentation as needed.

Level of Formal Education: Training from an accredited licenses or vocational nursing program

Special Certifications: Must have a Licensed Practical Nurse’s License current in the state in which they are practicing. Also, needs to be CPR Certified
