Good Shepherd Lutheran Home in Sauk Rapids, MN seeks a Full-Time Day Shift Licensed Practical Nurse (LPN) to provide skilled nursing care in Assisted Living. You will administer medications, monitor medical and psychosocial needs, and coordinate care under RN supervision.

The role includes leadership support when the RN is unavailable, staff guidance, and ensuring safety and comfort for residents across multiple care settings.

Competenze

• Graduate from an accredited school of professional nursing.
• Current Minnesota LPN licensure.
• At least 1 year of clinical experience preferred.
• Valid driver’s license required.

This position also provides nursing leadership and support to staff, coordinates client care, and helps ensure the safety, comfort, and well-being of clients throughout Good Shepherd's care settings.**Licensed Practical Nurse (LPN) Responsibilities:****Skilled Nursing & Client Care*** Provide skilled nursing services, including vital signs, focused assessments, and direct client care.* Monitor clients' medical, psychosocial, and physical needs.* Observe and report changes in clients' conditions to the RN.* Provide client education and reinforce appropriate care procedures.* Follow individualized plans of care and use proper medical asepsis.* Help prevent hazards of immobility and support clients' overall physical and psychological needs.* Coordinate client care in collaboration with the RN.* Complete ongoing client checks and rounding in assigned areas.* Provide assistance, reassurance, and purposeful one-to-one activity interventions during care and services.**Medication Administration & Treatments*** Administer medications and treatments according to physician orders and plans of care.* Transcribe and implement physician orders.* Prepare equipment and supplies for treatments.* Administer injections as ordered.* Order clients' medications and set up medications as directed.**Nursing Leadership & Staff Support*** Assume responsibility as team leader when the RN is not present.* Direct staff in appropriate procedures and delegated responsibilities.* Assist new nursing personnel with learning policies, procedures, and concepts of care.* Provide onboarding assistance to new staff to ensure they feel welcomed and understand their job functions.* Reinforce and provide education on proper procedures.* Provide backup assistance with staffing schedules for Shepherd of Grace, Assisted Living, and Cottages, helping ensure schedules are complete and adequately staffed as needed.**Care Coordination & Communication*** Arrange physician visits and clinic appointments as necessary.* Coordinate hospital transfers when needed.* Respond to emergencies at Shepherd of Grace, the Cottages, and assisted living locations as needed.* Communicate with healthcare providers, including home health agencies, the VA, hospice, physicians, and clinics.* Maintain communication with families.* Keep the RN updated on changes and matters of importance.**Licensed Practical Nurse (LPN) Benefits*** Generous Paid Time Off (Part-Time/Full-Time Eligible)* Earned Safe & Sick Time Off* FREE Option for Health, Dental, Vision & Life Insurance for Employee (Full-Time Eligible* 401K with Company Match* Employee Referral Bonus* Tuition Reimbursement & Scholarship Opportunities## Qualifications**Licensed Practical Nurse (LPN) Qualifications*** Have graduated from an accredited school of professional nursing.* Have current licensure as a Licensed Practical Nurse in the State of Minnesota.* At least one (1) year of clinical experience preferred.* Must have a current, valid Driver’s License* Exhibit professional behavior and attitudes.* Demonstrate the ability to make appropriate professional judgments.* Possess good interpersonal communication skills.* Computer skills necessary.Keywords: Geriatrics, Nurse, Nursing, Licensed Practical Nurse, LPN, Assisted Living Facility, ALF
