Responsibilities

• Observe clients and report changes in condition to RN.
• Communicate with physicians and NPs about changes needing medical intervention.
• Assist with ordering and setup of client medications.
• Monitor unlicensed staff and guide duties to ensure timely, respectful care.
• Assist in training new employees as directed.
• Identify emergencies and initiate emergency treatment when needed.

Licensed Practical Nurse - Assisted Living

Salary Range: $29.00 To $31.65 Hourly

SUMMARY

The Licensed Practical Nurse (LPN) - Assisted Living provides quality care for clients receiving Homecare services in an Assisted Living Building adheres to professional nursing standard and practices set forth by the Nurse Practice Act and ensures compliance with state and federal regulations including policies and practices set forth by Sholom.

Primary Responsibilities

• Observe clients and report any changes in physical, social or emotional condition. Report changes to the Registered Nurse.
• Communicate in a timely manner with physicians and nurse practitioners' changes of condition requiring medical interventions.
• Assist with ordering and setup of client medications.
• Monitor unlicensed staff and provide guidance to promote the completion of duties and responsibilities for clients in a timely, professional, respectful and dignified manner.
• Assist in the monitoring and evaluation of job performance of unlicensed staff in collaboration with the Registered Nurse.
• Assist in the training of new employees as directed.
• Identify and respond appropriately to emergencies. Initiate emergency treatment.

Education, Experience and Credentials

• Must be licensed as a Licensed Practical Nurse in the State of Minnesota and must maintain licensure.
• Current certification in CPR preferred; upon hire if not certified, must obtain no later than the completion of on the floor orientation.
• Two or more years of experience working with the geriatric population or in a similar care environment preferred.
