Job summary

Tucker House Nursing and Rehabilitation is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in Philadelphia, PA. As an LPN, you will provide direct patient care, administer medications, and support residents' daily living activities in a warm, homelike environment.

Qualifications

Responsibilities

About the Role

As an LPN at Tucker House, you will be responsible for providing direct patient care, including administering medications, wound care, and assisting with daily living activities. You will work closely with our skilled nursing team to develop and implement individualized care plans that meet the unique needs of each resident. Responsibilities

• Provide direct patient care, including administering medications, wound care, and assisting with daily living activities
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Observe and report changes in residents' conditions to the Registered Nurse or other healthcare professionals
• Participate in resident assessments, care planning, and care delivery
• Maintain accurate and complete records of patient care and treatments
• Collaborate with other healthcare professionals, including physicians, occupational therapists, and physical therapists, to deliver comprehensive care

Requirements

• Current LPN licensure in the state of Pennsylvania

• One year of previous experience as an LPN preferred
