Job summary

Tucker House Nursing and Rehabilitation is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in Philadelphia, PA. As an LPN, you will provide direct patient care, administer medications, and support residents' daily living activities in a warm, homelike environment.

You will collaborate with the interdisciplinary team to develop and implement individualized care plans, monitor residents closely, and maintain accurate documentation of care and treatments.

Qualifications

• Current LPN licensure in Pennsylvania.
• High school diploma or equivalent required.
• One year of LPN experience preferred.

Responsibilities

• Provide direct patient care, including administering medications and wound care.
• Assist with daily living activities and care planning as part of the interdisciplinary team.
• Observe, report and document changes in residents' conditions to nursing staff.
• Participate in resident assessments, care planning, and care delivery.
• Maintain accurate records of patient care and treatments.
• Collaborate with physicians, therapists, and other clinicians to deliver comprehensive care.
