Job summary

Heritage Woods of Rockford is seeking a Licensed Practical Nurse (LPN) for a Monday–Friday 9:30am–6:00pm shift, with rotating weekend coverage. The role focuses on medication administration, individualized care plans, and promoting resident dignity and independence.

You will collaborate with physicians and staff to support residents’ physical, mental, and social well-being while engaging in ongoing education and quality improvement initiatives.

Qualifications

• Current LPN licensure in the state.
• CPR certification required.
• Excellent communication and interpersonal skills.

Responsibilities

• Oversees medication distribution as ordered by physicians.
