Heritage Woods of Rockford is seeking a Licensed Practical Nurse (LPN) to join our team on a full-time, first-shift schedule (Mon-Fri 9:30am–6:00pm) with rotating weekends in Rockford, IL. You will ensure safe medication administration and develop individualized care plans to support resident independence and dignity.

The role emphasizes collaboration with physicians, families, and staff to promote resident safety and well-being.

Qualifications

• Current state licensure as Licensed Practical Nurse (LPN).
• Current CPR certification.
• Exceptional communication and interpersonal skills.
• Must demonstrate the ability to serve with love, compassion, and dignity.

Responsibilities

• Oversees medication distribution as stated on physician orders, ensuring adherence to medication management documentation and resident safety.
• Upon thorough assessment, creates personalized service plans tailored to individual care needs while championing resident independence and dignity.
• Cultivates communication between physicians, healthcare providers, and family, fostering an effective exchange of information pertaining to residents’ well-being.
• Contributes toward quality assurance and performance improvement initiatives.
• Engages in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences.
