Heritage Woods of Rockford in Rockford, IL is seeking a part-time Licensed Practical Nurse (LPN) to oversee medication distribution per physician orders and uphold resident safety with compassion and dignity.

You will collaborate with physicians, nurses, families, and other professionals to develop personalized care plans, support resident independence, and participate in quality improvement efforts while engaging in ongoing learning and attending educational sessions.

Qualifications

• Current LPN licensure in Illinois.
• CPR certification is required.
• Excellent communication and interpersonal skills.
• Ability to serve with love, compassion, and dignity.

Responsibilities

• Oversees medication distribution as stated on physician orders, ensuring strict adherence to medication management procedures and resident safety.
• Creates personalized service plans tailored to individual care needs while promoting independence and dignity.
• Facilitates communication between physicians, healthcare providers, and family regarding residents' well-being.
• Contributes to quality assurance and performance improvement initiatives.
• Engages in ongoing learning via seminars, staff meetings, and other trainings.
