Gardant-Management-Solution in Rockford, IL seeks an LPN to oversee medication distribution per physician orders and ensure resident safety. You will assess residents and create personalized service plans that promote independence and dignity.

You will liaise with physicians, healthcare providers, and families to discuss physical, mental, and social well-being, and participate in quality improvement initiatives and ongoing education opportunities.

Qualifications

• Current state licensure as Licensed Practical Nurse (LPN).
• CPR certification is required.
• Exceptional communication and interpersonal skills.
