Job summary

Springdale Health and Rehabilitation Center is seeking an experienced LPN for the night shift. You will provide direct nursing care, assist with rehabilitation plans, and collaborate with RNs, physicians, and the interdisciplinary team to ensure high-quality care in a long-term care setting.

Required are a current LPN license and BLS/CPR certification, with strong clinical skills in meds and wound care. The role emphasizes teamwork, compassion, and adherence to care plans.

Qualifications

• Current and valid LPN license.
• BLS/CPR certification required.
• Strong clinical skills, including medication administration and wound care.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents’ health status, including vital signs, and report changes to the supervising RN or physician.
• Assist with activities of daily living (ADLs) including bathing, dressing, and feeding as needed.
• Collaborate with the rehabilitation team to support residents’ recovery goals, including PT/OT/SLT plans.
• Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations.
• Provide emotional support and education to residents and their families.
• Ensure a safe and clean environment, adhering to infection control protocols.

In this role, you will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with RNs, physicians, and interdisciplinary team members to ensure the highest standards of care in our long-term care and rehabilitation setting.
