Job summary

Oak Knoll Health and Rehabilitation seeks a skilled Licensed Practical Nurse (LPN) to provide direct nursing care to residents in a long-term care setting. You will monitor health status, administer medications, and assist with rehabilitation plans in collaboration with RNs and physicians.

Qualifications

• Current and valid LPN license.
• Strong clinical skills including medication administration and wound care.
• Excellent communication and interpersonal skills.

• BLS/CPR certification required.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents’ health status, including vital signs, and report changes.
• Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding.
• Collaborate with rehabilitation team on recovery goals.
• Maintain accurate documentation per regulations.
• Provide emotional support and education to residents and families.
• Ensure a safe, clean environment adhering to infection control protocols.

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. In this role, you will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with RNs, physicians, and interdisciplinary team members to ensure the highest standards of care in our long-term care and rehabilitation setting.

• Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations.
• Provide emotional support and education to residents and their families.
• Ensure a safe and clean environment, adhering to infection control protocols.
