Job summary

Select Home Health & Hospice, a Fortis Health Company, in Indiana is seeking a compassionate Nurse Practitioner to join our hospice team. You will perform face-to-face hospice visits, coordinate patient plans of care, and collaborate with physicians and IDG members to deliver high-quality palliative care.

Qualifications

• Minimum 5 years of nursing experience (preferred).
• Current NP or CNS certification in Indiana.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct required face-to-face hospice visits and report findings to the Hospice Medical Director.
• Develop, coordinate, and document patient plans of care.
• Collaborate with attending physicians and IDG members.
• Prescribe medications within scope and delegation.
• Educate patients, families, staff, and community on hospice and palliative care.
• Maintain accurate clinical documentation for compliance and reimbursement.
• Supervise and evaluate home health aides as appropriate.
