Job summary

Spring Valley Health and Rehabilitation is seeking a compassionate Licensed Practical Nurse (LPN) to join our team. You will provide patient-centered nursing care, implement physician orders, and support care plans across shifts.

We offer a $10,000 sign-on bonus for full-time, with starting pay at $32.00 per hour, shift differentials, PTO, holidays with premium pay, tuition reimbursement, 401(k) contributions, and comprehensive benefits.

Qualifications

• Graduate of accredited LPN School of Nursing.
• One year of nursing experience is preferred.
• Valid LPN license.
• Valid CPR certification.
• Candidates must maintain a working email address and phone number for employer communication.
• Ability to work cooperatively as a member of a team.
• Ability to communicate effectively with residents and their family members, and at all levels of the organization.
• Ability to react decisively and quickly in emergency situations.
• Ability to maintain confidentiality.
• Knowledge of training techniques for clinical staff.

Responsibilities

• Assist in all aspects of nursing per care plans.
• Initiate and lead individualized nursing care plans.
• Assess and document residents' condition and needs.
• Accurately implement physicians’ orders.
• Assign nursing care to team members.
• Administer medications.
• Start IVs and perform treatments as needed.
• Maintain supplies and equipment in a safe manner.
• Other duties as assigned.

• Days 6:30 AM - 6:30 PM
• Nights 6:30 PM - 6:30 AM

Essential Functions of Licensed Practical Nurse (LPN):

• Highly motivated and able to assist in all aspect of nursing
• The candidate will provide services that exceed company and regulatory standards
• Initiates and leads individualized nursing care plans
• Assesses and documents the resident’s condition and nursing needs
• Accurately and promptly implements physicians’ orders
• Assigns nursing care to team members in accordance with the resident’s needs and the person’s capabilities and qualifications
• Administers medications
• Starts IVs and performs treatments for assigned residents
• Documents that treatment as required
• Ensures that supplies are utilized economically, and that equipment is clean and maintained a safe manner
• Others duties as assigned
